1. General and nature of the service

These terms apply to the use of tevakolhealth.com and to the coordination services of Tevakol Health, operated by [name of the operating entity, registration number and address — to be completed].

Tevakol Health acts solely as a medical-travel coordinator / intermediary: coordinating contact with hospitals and clinics in Azerbaijan, logistics planning and personal accompaniment. We are not a medical care provider, we are not doctors and we are not a hospital, and we do not perform treatments.

Using the website or the service means you accept these terms. If you do not agree, please do not use them.

2. Patient declarations

By contacting us and using the service you declare that: (a) you are 18 or over, or the enquiry is made by a parent or legal guardian; (b) all the information you provide, including medical information, is accurate, complete and up to date, and you will inform us of any change; (c) you have consulted, or will consult before any decision, a licensed physician in Israel; (d) you understand that every medical, surgical or aesthetic treatment carries risks, including complications, and that the decision whether to undergo treatment is yours alone — the doctor advises, the patient decides.

3. Independent third-party providers

Medical treatment is provided solely by the hospital or clinic in Azerbaijan and its doctors. They are independent parties, not employees or agents of Tevakol Health; the medical treatment agreement is made directly between you and them, and they bear the medical responsibility.

Airlines, accommodation, transfer providers and insurers are also independent third parties acting under their own terms.

4. No guarantee of results, availability or prices

We do not guarantee any medical or aesthetic result. We do not guarantee the availability of any doctor, clinic, date, flight or accommodation.

Price ranges and quotes are based on information provided by the hospital or clinic and may change, including after a medical examination. The binding price is the one in the final written proposal.

5. Coordination fee and payment

Any coordination fee will be set out in advance and in writing in the personal proposal: [amount / rate of the coordination fee and payment terms — to be completed].

6. Cancellation policy

A coordination transaction made at a distance (by phone, WhatsApp, the website or email) may be cancelled within 14 days of the transaction or of receiving the disclosure document, whichever is later, subject to the Israeli Consumer Protection Law, 5741-1981, and the Consumer Protection Regulations (Cancellation of Transaction), 5771-2010, and to the conditions below.

7. Limitation of liability

To the maximum extent permitted by law, Tevakol Health is not liable for medical outcomes, complications, the quality of treatment or any medical damage, direct or indirect, arising from treatment provided by a third party.

In any event, Tevakol Health's total liability in connection with the service, on any ground, shall not exceed the coordination fee actually paid to it for that service. Nothing here limits liability that cannot be limited by law.

8. Indemnification

You agree to indemnify Tevakol Health for any damage, loss or reasonable expense, including legal fees, caused to it by your providing incorrect, incomplete or misleading information, or by your breach of these terms. [Scope and conditions of the indemnity — for lawyer review].

9. Force majeure

We are not responsible for delay or non-performance caused by circumstances beyond our control, including war, hostilities, terrorism, epidemic, natural disaster, strikes, airspace closures, flight cancellations, decisions of authorities or infrastructure failures. In such cases we will try, as far as possible, to coordinate alternatives.

10. Organ transplants prohibited

Tevakol Health does not engage in, or act as an intermediary for, organ transplants of any kind, and does not coordinate any activity related to organ trafficking. Enquiries on this subject will not be handled. (Treatments such as hair transplants or dental implants are not organ transplants.)

11. Information on the website

The information on the website, in the chat and in any correspondence is general only and does not constitute medical advice, diagnosis or a treatment recommendation. The website chat is powered by an AI tool and may make mistakes.

12. Governing law, jurisdiction and language

These terms and any matter arising from them are governed solely by the laws of the State of Israel. Exclusive jurisdiction lies with the competent courts in Israel [district / city — to be completed].
